08/13/2013 This information is for the
cms1500 0212 form
to possibly be required between 01/01/2014 and 10/1/2014
This new form is similar to the previous cms1500
form 0805. There are many changes, however, the main change is the ability to
accept up to 12 ICD10 diagnosis codes in box 21.
To use the 0212 form, you must have Stratford
version 9.4.8338 or later
1. select #8 from the main menu, then insurance
control, enter your form type (example: 511).
2. See the 2 text boxes in the upper left. Right
click on 'type of form' and select mcds0212.
3. If you want the software to convert your ICD9
codes to ICD10 codes, right click on the diagnosis text box and select ICD10.
This will cause your form to have ICD10 diagnosis codes regardless of what you
entered in your patient data for TEST CLAIMS ONLY PRIOR TO 10/1/2015.

1. January 6, 2014 payers
begin processing the 0212 form
2. January 6, 2014 through
March 31, 2014 payers will process both the 0805 (old) and the 0212 (new) form
3. April 1, 2014 payers will
only process the new form 0212
